We are pleased you have chosen our services and will do everything possible to optimize your satisfaction while you are here. Listed below are some policies and suggestions we have in place while you are receiving physical therapy. If you have any questions, concerns or comments, you may inquire with our front office staff to speak with the clinic manager or request to speak with the owner.

To begin your treatment and rehabilitation with Peak Motion, we need you to provide us with some basic background information. Please download the Peak Motion Registration Packet, which includes four documents. Complete the information and bring this material with you for your first appointment.

When you arrive for your appointment, please sign in and let the front office staff know you are here for your evaluation. They will take your paperwork and go over your insurance benefits with you, which will include deductible, copayment and/or coinsurance information. We will collect any payments up front, before your treatment begins.

Your first appointment (evaluation) should take about an hour to an hour and a half. A Physical Therapist will discuss your therapy plans with you and put together a treatment plan for your follow up visits. Wear comfortable clothing with sneakers, and you are welcome to bring something to drink, although we do have a water station.

NOTICE FOR PATIENTS UNDER AGE 18: We will not schedule an underage patient until the insurance benefits are discussed with and acknowledged by the parent(s)/guardian(s). It is our policy that a parent/guardian of an underage patient be present during the first visit (evaluation) to our clinic.
